How to integrate data?
DISI refers to the practice of implementing secure health data exchange in a health information exchange (HIE) and then centralizing and integrating HIV data in a central repository.
DISI enables patient records to be submitted by different health facilities or systems. These records are stored centrally in a national Data Repository in order to reduce and potentially eliminate duplicate patient records with the use of a Client Registry system.
This enables healthcare professionals to effectively measure and monitor the progression and outcome of HIV through case-based surveillance (CBS) sentinel events.
How are we leveraging existing platforms (Client Registry, Interoperability Layer, analytics, and visualization, etc.) and products (standards, HIE architecture, etc.) to demonstrate DISI?
DISI leverages existing platforms/solutions, such as point of service (PoS) applications, centralized repositories for strengthened case surveillance, and adaptable analytics specifications, to facilitate the exchange and integration of patient care and treatment information.
- DISI implements key components to enable the secure transmission of messages between the PoS client, interoperability layer, Client Registry, Data Repository, and the analytics and visualization tool.
- The DISI centralized architecture aims to reduce and potentially eliminate duplicate HIV CBS patient records submitted by different health facilities or systems in a country.
- The DISI centralized architecture also ensures the overall quality of longitudinal clinical data which helps HIV clinicians, case managers, and country leaders effectively monitor the HIV epidemic.
- Using specialized reporting mechanisms and visualization tools, authorized users will be able to produce case-based surveillance, patient care coordination, and program monitoring reports.
For sustainable implementation, DISI supplements technical products with the process, standards, guidance, and learning tools and materials to support in-country users in the customization, implementation, monitoring, and assessment of data integration standards and strategies.
